If the request is considered for approval, the applicant will be contacted via email with the applicable licensing fee, terms, and further steps required to obtain the official license. No rights are granted until all licensing fees are agreed upon in writing and paid in full.

By signing below, I acknowledge and agree that: All artwork created by Tamara Cooper, under Tamara's Photo Art Illustrations, is protected under United States copyright law. Ownership of the physical artwork does NOT transfer copyright ownership. No reproduction, resale, distribution, modification, or commercial use is permitted without written authorization. Any approved use will be subject to a separate written licensing agreement and possible licensing fees. Unauthorized use may result in legal action. I understand that submission of this form does not guarantee approval. Permission will only be considered valid if it is granted in writing via email directly from Tamara Cooper, owner of Tamara's Photo Art Illustrations.
